Please help me if you can. In the attached picture:

1. What are these two adjustment screws used for? what do they adjust?

2. If you have a Grizzly LX, have you ever had them come loose? Back-out on their own?

Those are for Upper Wheel Lateral Adjustment – an adjustment to make the wheels co-planer or in alignment. Look on page 49 of your manual. -Jack
