Quick Release Vice problem

I have a Groz quick release vice that has jammed up in the closed position. This was proceeded, for quite some time, by the quick release bar slipping on me whenever I loosened the vice. I removed all the quick release parts and found that the threads inside the offset cam block (middle item in photo) were all junked up with grease and dirt so I cleaned it. I made the mistake of not taking a picture of the assembled parts and am now having trouble reassembling. It seems to go together properly only one way—and I’ve done that—but it didn’t work at all. The handle just spun without engaging. Attached is a photo of the three parts I need to reassemble. Is anyone familier with this particular quick release? Any help would be appreciated.
