Not so sound pesky, but a simple Google search should answer your question.

It appears to be a custom made door most likely ordered in bulk by the builder.

Full length louvered doors are readily available at HD, Lowes, etc. but they are interior doors and they are not split like yours appears to be.

I don’t see why they need to be louvered so call your utility company.

I did Google but nothing useful resulted. Also checked at the local big box stores and one specialty wood supply store and nothing there either that addresses requirements I posted above.

From what I found so far the lower smaller door that encloses the gas meter may need some amount of ventilation. The upper for electric meter has no venting requirement.

Not that hard to get just any door that functions but there is the added need to have the uniform appearance of original build, which is louvered look. I know there is a whole world of good info that Google will not catch which is why I posted here.

Whether you can change the look or not would depend on how picky your HOA is..

Might be a cool project to try and tackle yourself with some of that Trex (other brands available) decking.
It’s basically plastic but can be milled the same as regular wood. It’s pricey, but would never rot again.

And if it turns out okay, you might have some income replacing your neighbors doors too
