I have used Keim Lumber in Charm Ohio for years. I recently had them make long, glued-up countertops for all of the cabinets in my garage. They are not cheap, but they can custom make just about anything. In addition to a huge variety of suppliers, they manufacture their own line of doors (I would love to use their quarter-sawn white oak doors in a new home some day) and custom mill any type of woodwork.

They have their own machine shop that will cut planer knives to match any moulding you need. It may be worth a try. I have been able to e-mail them drawings and specs and get a quote back.
