Is the plate tight fitting in the table top? perhaps a slight movement of the plate relative to the table combined with a slight lateral movement between router base and plate has shown obvious off center alignment.

Have you tried removing both plate from table and router base from the plate, then resetting/adjusting. Perhaps it just needs some maintenance, by tightening all the adjustment parts.

Also worth checking the router base to see if the screw threads have worn causing slippage, I’m sure it’s something simple.
