Your wheel doesn’t look like it on far enough. Make sure the set screw is loose and take a mallet and block of wood and try to tap the wheel on farther. Take a something with a long straight edge and place it across the face of the wheels from the top of the top wheel to the bottom of the bottom wheel. The straight edge should make contact or be very close on all the edges of the wheels if they are on far enough. Then tighten the set screw.

The wheels don’t have to be perfectly coplanar but they should be fairly close.
